221 Social Security Numbers (SSN)

With few exceptions, all applicants and recipients who request or receive medical assistance must provide their correct Social Security number.  A Social Security card or other verification of the Social Security number is not required unless the number cannot be verified by computer match.  If an applicant does not have a Social Security number he or she must apply for one and provide the number once one has been assigned.
