221-1 Verification of the Social Security Number

No Previous Policy

Once a Social Security number (SSN) is provided by the applicant or recipient it must be verified as the correct number for that person.

Verification of the Social Security Number by Computer Match

First check to see if the SSN has already been verified by computer match. If the SSN has already been verified by the computer match, no other verification is needed.

If the SSN is not known, the computer match system will attempt to verify it with Social Security records once the number has been entered into the system.

Do not deny, delay or discontinue medical assistance while waiting for the computer match verification.

Verification of Social Security Numbers That Do Not Match

At review, check to see if a computer match has verified the SSN. If the SSN number has not been verified  require the person to provide hard copy verification of the Social Security number.  Any one of the following forms of verification is acceptable:

An official Social Security card; OR

Other official documents from the Social Security Administration (SSA) which identify the correct SSN.  These include, but are not limited to, award letters,  and benefit checks; OR

A form 5028 or 2880 from the Social Security Administration which includes the SSN.

Individuals who fail to provide verification of their SSN after being informed of a computer match error are not eligible.  Provide 10-day advance notice before closing a case or removing a person from the coverage.
