343-6 Reopening a 12 Month Transitional Case

Reopen 12 Month Transitional when the household:

Provides the report by the last day of the report month. If the last day of the report month is a Non-business DaySaturday, Sunday and state holidays., the household has until the following Business DayFor the purposes of medical assistance eligibility, business day means any Monday, Tuesday, Wednesday, Thursday or Friday that is not a state holiday. to provide the report. The household does not have to have good cause for a late report.

Provides the report after either the last day of the report month or the Business DayFor the purposes of medical assistance eligibility, business day means any Monday, Tuesday, Wednesday, Thursday or Friday that is not a state holiday. following the last day if the last day of the month is a Non-business DaySaturday, Sunday and state holidays. and the household had good cause for late report.  (343-4.)

Moved out of the state then moved back to Utah during the 12 month period and reapplies or requests the case be reopened. The household must provide all income and expense verification for any income report that would have been due during the time the case was closed.

Had the 12 Month Transitional close because the parent or caretaker relative was eligible again for PCR Medicaid and the children were eligible for child Medicaid, then:

o   The parent or caretaker relative lost eligibility for PCR, AND

o   did not qualify for a new 12 Month Transitional period, AND

o   There are still months remaining in the original 12- month period.
