A. Community Transitions Waiver (CTW)
CTW provides home and community based services in community settings for eligible individuals who would otherwise require the level of care provided in an intermediate care facility. The primary goal of the CTW is to move people out of institutional care to a less restrictive community care setting.
This waiver is for people of all ages who have intellectual disabilities and other related conditions and need help with activities of daily living. People on this waiver must meet an aged, blind or disabled Medicaid category.
