A. To be eligible, an individual must be screened for breast or cervical cancer by Utah Cancer Control Program (UCCP) and found to need treatment for breast or cervical cancer, or a precancerous condition. Individuals who have another type of cancer may be eligible if the primary cancer was breast or cervical cancer.
B. When an individual needs to be screened or has received a diagnosis from a medical provider other than a CDC contracted provider, refer them to UCCP to be screened for the Medicaid Cancer Program. The toll-free number is 1-800-717-1811.
C. UCCP will complete and sign a Medicaid Cancer Program Referral, Form 928, to verify that an individual has been screened and found to need treatment for breast or cervical cancer, or a pre-cancerous condition.
D. An individual who has moved to Utah can be eligible for the Medicaid Cancer Program if they were diagnosed by a CDC provider. Have the CDC provider verify:
· If the person still needs treatment.
· If they have breast or cervical cancer or a pre-cancerous condition.
· The diagnosis date.
